Mainstream acceted a new vhost patchset recently

That patchset supports creation and handling of multiple vhost workers, so
we can now drop our homebrew solution.
Unfortunately only a few patches from patchset was merged into new RHEL tree.
This series cherry-picks the most of ms patches, except for the new worker
managing API and vhost-net, scsi, etc. upgrades. We will wait till RH decide
to merge them all.
>From our old patchset we will use vhost-blk patch itself and API patch.
